Where are claims mailed?

All medical billing services and claims should be mailed or filed electronically by your provider to the address on the the back of your ID card.  SBMA claims should be mailed to:

Who should my provider or I call if I am going to be admitted to the hospital?

To pre-certify a hospital admission, contact the PRE-CERTIFICATION phone number listed on the back of your ID card.


 

Is Conifer Health Solutions an insurance company?

Conifer Health Solutions is not a health plan and they do not underwrite risk.  The major advantage of Conifer Health Solutions is our “portability”. Clients can use the Conifer Health Solutions managed care system with their insurance.
